Louise, I did a search at amazon.com on "designing surveys" and came up with
several books. This one sounds the most promising as a general reference:

Designing Surveys That Work : A Step-By-Step Guide
by Susan J. Thomas

Unfortunately, it's brand new (copyright May 1999) and hasn't been released
yet. However, amazon is taking pre-orders. With a copyright date of last
month, I imagine it should be released any day now.

This one, below, also sounds promising based on the title. However it is out
of print. Maybe you can locate a used copy:
