Narrator barely qualifies as a screen reader.

Window-Eyes, Jaws, Hal, and IBM Home Page Reader all work fine with 
tagged PDFs and OK with some untagged PDFs. Adobe (Acrobat) Reader 
has been self-voicing since version 6.
