Hi everyone - this is slightly specialised I guess but
I've put a draft of my handbook of incident and accident
reporting on:

It includes sections on the role of individual and team
based performance in adverse events, covering human factors
and some HCI related issues.   It also covers aspects of
bias that make witness statements difficult to interpret
and can affect incident analysis.

I hope it's of some interest to some of you - there isn't
enough HCI input in this area and interaction issues are an
increasinglty important aspect of accident investigation.
