I addressed this for a client in exactly that way: a unique item, once
added to a cart, is simply not shown to anyone else. A session timeout
results in the cart contents being added back to the inventory.

It's really not technically difficult and I totally agree that it's
"not good to surprise and annoy customers" 😀
