This is *not* best practice anymore. Even if older screen readers get it
wrong, newer ones get it right, and having a blank space is confusing.
It is best go with the 'null' alt of: <img src="spacer.gif" alt="" /> 

The RNIB (http://rnib.org/) use this method, and one of their prime
audiences are is the visually impaired. (I wouldn't look there for
best-practice on other aspects of HTML or accessibility though.)
