This sounds like a summative, baselining type of test given the  
metrics you have outlined.   You could start with the ISO standard  
usability test format:
ISO/IEC 25062:2006
Software engineering -- Software product Quality Requirements and  
Evaluation (SQuaRE) -- Common Industry Format (CIF) for usability test
