I second the suggestion to keep the questions on the same page and numbering
them with 6a, 6b and 6c.  If these optional questions are of some
significance to the company, it's unlikely you'll get too many responses if
they are on a different page. Besides, a multi-page registration form could
be a major barrier to acquisition.

To keep the form from looking too unwieldy or cluttered, you could hide or
gray out the 2nd and 3rd questions until the 1st question is addressed.
